Then The Exam Begins..

Dr. Es : Plz examine this patient's lower limb neurology system..

A'a : Alright Dr..

Dr. Es : but 1st i want the patient to walk around..in the ward...

sekali ingatkan jalan dekat2 situ jer..rupanya Dr. Es mintak patient tu jalan sampai ke depan pintu ward..alamak..nk g mana ni..tertanya dlm hati la..

"Pakcik..jalan lagi..kita gi round depan lift yer.."..Dr Es arahkan..

jauh benar tu..so, kami bertiga berjalan la sampai ke dalam lift..tp aA kena jadi obesrever yg sangat peka n kena analysis setiap yg abnormal dlm gait pakcik tu..susah wooo..dlm borderline antalgic dgn short limb gait gtu la...

tp Dr. Es baik..dia xmarah pun..ahhaha...

then patient balik ke katil semula...bermulalah semula exam td..

1. Inspect dulu the attitude, wasting, skin changes, scar etc kat lower limb..itu semua bg klu untuk tahu which level of the spinal cord or nerve root involve..

contoh..alo nmpak thigh muscle yg wasted..kena la tahu nerve root mana yg supply that muscle..

2. Then palpate the consistency of the muscle bulk..maybe nampak wasting tp bila palpate muscle bulk tu mungkin firm..tak soft...besides..check for any tenderness over the lower limb region..kekadang ada yg kalo spinal cord compression, even light touch pun diorg akan rasa sakit yg amat...

Oppss...terlupa..kena test untuk Straight Leg Raising Test and Sciatic Nerve strecth test gak..

then baru wat...

3. Check for the tone of the limbs

untuk lower limbs..kita rolled the limbs untuk check tone thigh muscle and muscle that support the hip joint..tgk kalo2 foot dia nampak senang bergolek ke tak...

then untuk check muscle lain kat lower limb...flex the hip and knee...pegang lutut patient and tapak kaki patient..rotate the limb...

kena tahu beza between normotonia, hypotonia and hypertonia...

4. Check for the power of the muscles...

- based on grading 5/5 or 4/5..and etc la...so, kena la main2 tarik n tolak kaki...

time nk assess ni kena betul2..xleh kekok nk pegang kaki patient or end up patient tersepak kita..

5. Reflex...hahaha...ni guna tendon hammer..memang masih kekok pun nk pegang..

bila pukul..nmpak jer reflex tp xkuat la..

sekali Dr. Es tunjuk kat Aa camner nk pukul...mak oiiii...kuatnya..

masa tu aA terkeluar ayat dpn Dr.... "Dr..kuatnya Dr. pukul..."...

then dia kata "bukan kuat, awak yg xconfident nk pukul kuat..."....hahaha...kena atas hidung...

lepas tu dia bg peluang buat lagi sekali...

berjaya..naik jer lutut tu rupanya..

6. Sensory...

test guna tangan jer..dah xingat dh nk guna pin prick or cotton..hahah..

sepanjang test tu..kalo ada technique yg xbetul..Dr. akan gtau and mintak aA perform semula...

ni kira 2nd time aA dapat kelas neurology individually dengan specialist..dulu masa exam Medical...

hbh exam td..lepas dpt locate the exact level spinal cord yg kena tu...rasa lebih yakin n confident untuk praktis lower limb neuro examination...

TQ TQ untuk Dr. yg cute..dan jugak kwn2 yg bagi semangat kat aA td..hehehe....

memang sgt lucky la coz dapat Dr. yg baik as examiner..semoga time exam final pun..aA mampu buat yg lebih baik..AMIN..

sebenarnya semlm lepas tulis blog kejap..aA try study..tp sebab penat sgt..xleh nk fokus..last2 tertido kat meja sampai pagi..kul 6.30 terjaga balik..sambung baca buku sampai kul 8..8.30am exam...

huhuhu...

bagaimanapun...aA xrasa stress dgn MCQ td..rasa hati..alhamdulillah boleh jawab gak..even tahu ada yg xleh jawab gak..
